USASK COVID-19 Lab Scheduling

If you would like access to any of the Pulse Research Group labs:

  1. Read the guidelines set out below and ensure you understand what is required of you. If you have any questions or concerns, please contact your supervisor or the lab manager for the lab you are requesting access to.
  2. Submit a Lab Time Request by filling out this form. This should only be completed for lab work. All analysis should still take place at home.
  3. Check back on your Lab Time Request to answer any questions lab management may have.
  4. Only enter the lab once your request has been approved.

YOU MUST FOLLOW THESE GUIDELINES AT ALL TIMES:

REMEMBER: Please enter the Agriculture Building through designated staircases as outlined in Occupancy Guidelines June19. You can only work in the spaces and for the hours you have been approved for, and after you have read all relevant documents and guidelines, and have returned a signed copy of the employee authorization form to your supervisor. Permits are posted on lab doors that list authorized workers, the maximum number of workers allowed in the lab and the allowed working hours. You can only enter the lab if the permit with your name on it has been posted at the lab door. If you were given permission to work after hours, your Working Alone Form also has to be posted to the door of the lab(s). Every authorized worker must fill in the Health Monitoring checklist and the Usask work location log on a daily basis. Each lab must have, and fill in on a daily basis, a USASK-cleaning-disinfectant log. Authorized workers MUST have signed on to Alertus: www.usask.ca/usafe.
